I have seen these here in the UK. If it is what I am thinking of, it is like a little plastic spout one end with a sharp pointed spout like tip on the other end with a bit of a thread that screws into the carton to hold it in place. They tend to leak though, I cannot imagine how you will solve that problem without some kind of glue or sealant. A peel off sticky(think double sided sticky tape) edge facing the carton might work but the glue would obviously need to be safe as some may disolve into milk.
Would this be aimed at the grocers or to the public to enable them to use this type of milk container? Only if you are talking about what I think then they would be a bit big for most people. The one's I have seen are like milk in a plastic bag inside a cardboard box only they usually have a tap at the bottom to release the milk. Usually into catering size vats etc.
I've heard my in-laws talk about seeing these milk bags when they traveled through Europe. They said how strange it was to see warm bags of milk stacked on the grocer's shelf. They had been irradiated so refridgeration was unnecessary. I think the spout idea is perfect. Cheap to produce and very simple and useful.
I'd really like to see a sketch of your design. Also, it's hard to imagine what the problem really is with this type of jug, since I've never tried one (or seen one before now).

@ Matt I believe this is the 'trinket' that you're talking about. And I don't think that this is anywhere near revolutionary.
Finally an idea that is revolutionary. Though quirky users would rather vote for trinkets that will make us no money.
A gasket like those used in sinks would be fine to seal the spout.
